About Silver Health CARE

Policies

Front Desk

To insure accurate and timely payment by your insurance company, it is very important that the receptionist:

  • verify your information at each visit
  • have copies of your insurance cards
  • collect your co-pay at time of service
  • collect referrals

When there are changes in your information, it can cause a delay in payment or an inaccurate bill sent to the patient. It is also a requirement (by your insurance) of physician offices to collect co-pays at time of service. The steps that we take at the reception desk are to insure that your business with this office is accurate and as stress free as possible.

Payment and Discount Information

For our patients who have no insurance, we offer a 40% discount when the visit is paid in full at the time of service.

For our patients who have a high deductible, over $1,000.00, we offer a 25% discount. This discount will be given when the visit is paid in full at the time of service.

This also applies to our patients who have been in the hospital, and who have no insurance. They are given the same discount if they pay the balance in full within 2 weeks of the initial billing.

Silver Health CARE "No Show" Policy

In fairness to all of the patient's and practitioners at Silver Health CARE, Silver Health CARE will now enforce a "No Show" policy. The practitioners of Silver Health CARE are very busy. Many patients have experienced difficulty scheduling necessary appointments. The purpose of this policy is to allow our practitioners to see as many patients as possible, so they can provide timely care for all our patients.

A "No Show" is defined as an appointment:

  • that is not kept and not cancelled more than 24 hours in advance.
  • that you are more than 10 minutes late for, unless the practitioner agrees to see you.

Failure to cancel an appointment at least 24 hours in advance will result in a $25.00 "No Show" fee.

The above fee must be paid prior to your next scheduled appointment. Insurance companies will not be billed for this fee. These penalties are accepted by insurance companies as an acceptable policy, but are the total obligation of the patient.

If you "No Show" more than 3 times per year without extenuating circumstances, you will be subject to dismissal from the practice.

Prescription Refill Policy

If you need a prescription refilled, contact your pharmacy and have them request the refill.

If you have not seen your doctor in over a year, you will need to make an appointment before your prescription can be refilled.

All local refill requests need to come from the pharmacy, not from the patient. Please allow two to three business days for the request to be completed.

Mail order prescriptions must be picked up at the clinic. We no longer fax prescriptions to mail order companies.
